I used my penfed at all damm gas stations across Johannesburg, Cape Town and Durban...Almost spent $2k on gas on a 25 day road trips...

The only difference I find there is you do pay at the pump but it is the cashier with a wireless swipe machine.
I was little suspicious and collected all the receipts. As expected, I never got any cash back. I made a claim and did send in all the receipts but finally it came out as declined saying that the gas has to be paid at pump.

Damm...at one point felt I lost a lot but was later happy that I did not pay any forex fees and I did get a very good conversion rate.

Interesting story but I'd say all bets are off once you go overseas on this sort of thing. And wireless swipe devices are specifically warned about with some cash back cards as not compatible with their checking system (Discover says this specifically).

Amex Costco does not give 4% on gas. Here are details from Amex website
Earn 3% Cash Back at US gas stations and on gasoline at Costco up to $4,000 per year in purchases (then 1%), 2% Cash Back at US restaurants, 2% Cash Back on eligible travel purchases, including at Costco, and 1% Cash Back on other purchases, including at Costco.

For what its worth the Cash+ gave me 2% for purchases made inside a Super Wawa (at the register, not the pump). But then again, it's only 2% and nothing to really get excited about considering 2% everything cards like the Priceline Visa or the Fidelity Amex.
