I'm really shocked, I thought US Bank would be one to offer great customer service and go above and beyond for the customer, not try and rip them off. OP, have you tried contacting US Bank through their Twitter: https://twitter.com/askusbank ?
I've found that the social media teams for various companies are typically able to provide better customer service.

But definitely press this issue with them, it definitely sounds like they were in the wrong.

FYI, what they said on the phone about not offering Cash+ outside of their area is not correct. I live in Massachusetts where they also have no branches and I successfully applied for and received a Cash+ card just last week. However, I did so in person on a business trip to Seattle where they do have branches. Maybe that helped.

Annoyingly, my wife was issued a Cash Rewards card instead, even though we asked the representative to not substitute any other card. :(

Their site currently states "Apply in branch only" for this card. While I agree that what US Bank did was fishy I'm not sure why the OP expected to be approved for this card given that information.

He apparently called and spoke to someone who did the application, so trying to lay the blame for the OP on him for not having read that is kind of a cop out. He should have every right to expect when he calls and speaks to a representative of the bank that they would be able to provide that information and apparently, they did not. That's on US Bank, not OP.

+1000 Social media requests get prompt and robust customer service. Tweet us bank or ask us bank and accuse them of the bait and switch. You'll get a message asking for your info so they can all you.

Just an update... I contacted U.S. Bank through their Cash+ Facebook page and a very helpful representative told me that I should have never been able to apply for the Cash+ card over the phone in the first place, but since I was told by the phone rep that I could, she had them switch me over to the Cash+ card and I just received it in the mail today.

So I finally got my Cash+ card. Now it's going to have to compete with my CSP for a spot in my wallet for my $700/mo restaurant spending...

I know three cards isn't a lot (see my sig), but I already feel overwhelmed with choices of what card to put charges on !

I do, however, think that I have the three best cards in the business. ;)

Congratulations! You do have 3 very nice cards it's all you really need....now as to what you want well that's another story.
