How to remove authorized user from credit card?

Hello all, I am brand new to the page. I'm hoping for some insight into a couple things.

First is how do I remove myself as an authorized user from someone else's account?
I was added as an authorized user about 2 years ago by an ex- girlfriend, but we haven't been together since last year, yet on my credit reports, her account shows up. I have tried disputing through the reporting bureaus to no avail.

Also I have a creditor named Audit Systems, which is a collection agency collecting dept for ADT security. I paid the collection account in full at the time I received the letter from them and then they went ahead, even after saying they wouldn't, reported negatively to the bureaus. I also have an LVNV account posting erroneously as a creditor and increasing fees by the $100's every month! Good news is, they are due to drop of by Jan 2014. I'm actually considering I send them a a FOAD letter as I despise their evil ways.

Your ex-girlfriend would have to call and have you removed. Only the owner of the credit card is allowed to remove an authorized user.

I think it actually varies per bank.
Where I work, an AU can call in and remove themselves from the account.
But only themselves, of course.
