A cardholder would have to spend $6500 to achieve the 5% threshold? Does this "grandfathered" card have a annual fee?

I don't have it, but my parents do. They really like it and they easily cross the $6500 spending.

What happens when you exceed the $6500 limit? Does the 5% kick-in or downgrade to only 1% on all categorical charges?

Yup. The link seems to come and go for me but if you click on View Quick Info for the Blue Cash Everyday there should be a link there that says "Looking for an earlier version of the Blue Cash Card?".

EDIT: Must be a cookie thing. Link isn't there for me with Chrome (browser I mainly use) but it is there for IE. It takes me to:
https://www304.americanexpress.com/credit-card/blue-cash/25330 (which doesn't work for me with Chrome)

Already explained above but one only gets 1% on "Everday Purchases" until one reaches $6,500. Then the 5% for "Everyday Purchases" kicks in with no limits. Other purchases are at 0.5% until $6,500 is reached and then 1% after.
