I was originally looking for some other information and stumbled on to this thread. Since it's still such a hot topic I thought I'd register to bring my 2 cents to the table.
Now I'm from the Netherlands so I can't comment on the experience of others, however.....
This is how we got our centurion membership:
To start with you should know Amex doesn't have a high acceptance rate here in many stores. The fees are rather steep compared to other payment methods i'll admit so other than the really high end and multi-nationals companies, not a lot of vendors use it.
We had been loyal Amex customers for years, importing IT stuff and software from the US and paying with a pretty basic card. We did have some limit increases from time to time.
Then got a call from their sales department asking us to accept their card in our store. Lo and behold out comes an invitation for the centurion membership as a perk to accept their card in our store.
Truth time: we said yes thank you to the centurion membership, then said no thank you to accepting the card.
